              e)   Certificate of Lung Foundation membership.

2. The Health Professional Membership ($66 including GST) confers the following benefits:

             a)    Attendance to Education Day(s) conducted in every state on an annual basis.

             b)   Eligibility to attend the annual general meeting and receive relevant papers.

             c)    Use of Lung Foundation member services. (to be implemented 2010)

             d)   Circulation of Lung Foundation newsletter – LungNet News.

             e)   Certificate of Lung Foundation membership

             f)     Opportunity to participate in relevant Lung Foundation Consultative Groups.

To receive your one year paid Australian Lung Foundation membership compliments of AIR LIQUIDE Healthcare, please print out and complete the attached  form and mail to the Australian Lung Foundation.   

Please note that the one year paid membership can only be supported by AIR LIQUIDE Healthcare for applications received by the Australian Lung Foundation with the our  logo appearing on the form.
