Successful treatment of obstructive sleep apnea (OSA) can improve all aspects of a person’s life. CPAP therapy is the ‘gold standard’ and most common treatment and does not involve drugs or surgery. Patients can experience sleep improvements within their first few nights.

Sleep apnea treatment options

CPAP (Continuous Positive Air Pressure) is the most common therapy for obstructive sleep apnea and is regarded as the ‘gold standard’ therapy because of its effectiveness across a wide range of cases. CPAP therapy

CPAP consists of a flow generator that pumps air through a tube into a mask worn by the patient. This increases the air pressure inside the patient’s throat, supporting the airway and keeping the path to the lungs open by preventing the throat muscles from collapsing into the airway.

Patients usually report that CPAP therapy improves their lifestyle dramatically.

Being able to breathe properly improves their sleep – leaving them feeling refreshed in the morning. Not snoring often improves their partner’s sleep too, with predictable relationship improvements. The CPAP mask is only worn during sleep and manufacturers go to great lengths to make the masks as comfortable as possible. Small, travel-sized CPAP devices are also available.

Mandibular Advancement Device (MAD)

While CPAP therapy is the ‘Gold Standard’ therapy for OSA, there are some patients who are unable to tolerate CPAP. Mandibular Advancement Device (MAD) has been proven to be as effective as CPAP in patients with mild to moderate OSA. While there are several options of MAD, including the simplest ‘boil and bite’, semi-custom and custom-made MAD, it has been shown that patients prefer the semi-custom MADs.  Semi-custom MADs are the most cost-effective device, with a marginal room for adjustment. 

Factors associated with improved outcomes with MADs include:

  • Lower severity of OSA (mild to moderate)
  • Female
  • Younger age group
  • Side sleepers
  • Receding jaw

Other treatment options

The biggest challenge to successful CPAP therapy is compliance with treatment. Patients can be uncomfortable or embarrassed using the device and wearing the mask early in their treatment when they have not yet experienced its full benefit. This reluctance can lead to inconsistent use, or abandonment, of the therapy.
